Calls from physician offices will be given priority based upon professional opinion for either “same day” or “next day” booking and promptly provide follow-up consult letters to referring physicians.

To schedule a new patient referral, please be prepared to give our office the following information:

• Date of birth
• Insurance carrier
• Subscriber name
• Insurance ID & Group number
• Patient contact number

We also request the report from the patient’s most recent clinic visit from your office and copies of any lab reports.
